For those interested.
The failure was caused by a hardware fault on the master database cluster. The fault manifested itself in such a way that the slave/failover cluster did not detect the failure of the master.
Once we we notified of the problem we immediately instructed the engineers to force the slave to sync with the logs from the master cluster and to take over.
We are looking at whether we can improve the monitoring of database clusters as a result of this problem.
